The trip is tentatively scheduled for Wednesday, May 13th.  We perform at the food court at noon, then chaperones take a group of students and spend some time in the mall.    Chaperones may ride the bus with us from school (leaving school at 11:15) or meet us at the mall at 11:30. 

 

School policy requires chaperones to have all appropriate volunteer clearances.   

 

 

 

These clearances are needed to volunteer for our school:      

 

• Pennsylvania Child Abuse History Clearances (CY113)

• Pennsylvania Criminal Record Checks (SP4-164)

• Federal Bureau of Investigations (FBI) Criminal Background Checks 

 

 

Notice:   If you have been a resident of Pennsylvania for the entirety of the previous ten year period, in place of the FBI Criminal Background Check you may print and sign this waiver.
